Anthem EAP maintains policies that protect the confidentiality of personal information obtained from its members in the course of its regular business functions. Anthem EAP is committed to protecting information about its members, especially the confidential nature of their personal information (PI).

Personal Information is information that is capable of being associated with an individual through one or more identifiers including but not limited to, a Social Security number, a driver’s license number, a state identification card number, an account number, a credit or debit card number, a passport number, an alien registration number or a health insurance identification number, and does not include publicly available information that is lawfully made available to the general public from federal, state or local government records or widely distributed media.

With your Anthem EAP registration, we will ask for personal information, such as first and last name, date of birth, etc. in order to better service your needs. Anthem EAP registration will allow you access to the Anthem EAP’s provider network information and request confirmation for use of services.

Upon registration, you will establish a user name and password through which you will access the application and use your user name and password each time you return to the website. You will log on through the Member Center. You can change your password at anytime if you feel that your password has been compromised.
